Myocarditis merupakan kondisi peradangan pada myocardium. Myocardium adalah lapisan tengah dinding jantung. Berbagai tanda dan gejala yang termasuk nyeri dada dan irama jantung abnormal dapat disebabkan oleh kondisi ini, namun jarang terjadi.

Dalam kasus myocarditis yang parah, aksi pemompaan jantung akan melemah dan jantung tidak akan mampu memasok seluruh tubuh dengan darah yang cukup. Hal tersebut dapat mengarah pada perkembangan serangan jantung atau stroke, dan dapat membentuk gumpalan dalam jantung.

Infeksi virus biasanya menyebabkan myocarditis. Pengobatan myocarditis yang dilakukan difokuskan pada penyebabnya.



Penyebab

Demam rematik merupakan penyebab

umum dari myocarditis, namun biasanya ada penyebab lain dari myocarditis. Myocarditis paling sering berkembang karena infeksi yang disebabkan, antara lain oleh:

1. Parasit
Toxoplasma dan Trypanosoma cruzi, termasuk beberapa parasit yang ditularkan oleh serangga dan dapat menyebabkan kondisi yang disebut penyakit Chagas. Penyebab yang paling umum dari gagal jantung kongestif adalah penyakit Chagas.

2. Bakteri
Termasuk bakteri tick-borne yang menyebabkan penyakit Lyme dan Staphylococcus aureus, yang juga dapat menyebabkan infeksi pada katup jantung yang disebut endocarditis. Lebih dari seperempat dari semua orang dengan infeksi bakteri difteri, juga terjadi myocarditis.

Gejala

Tergantung pada penyebab dan tingkat keparahan penyakit, tanda dan gejala myocarditis dapat bervariasi. Berikut ini adalah termasuk dalam tanda dan gejala paling umum:

1. Retensi cairan dengan pembengkakan pergelangan kaki dan kaki
2. Nyeri dada yang mungkin terasa seperti serangan jantung atau angina
3. Sesak napas pada saat istirahat atau selama aktivitas fisik
4. Kelelahan
5. Denyut jantung yang cepat atau abnormal (aritmia)

Sedangkan tanda dan gejala yang kurang umum terjadi, antara lain:

1. Pingsan atau tiba-tiba kehilangan kesadaran
2. Gejala lainnya yang terkait dengan infeksi virus seperti demam, nyeri sendi, diare, sakit kepala, sakit tenggorokan, atau nyeri tubuh.

Pericarditis, yang merupakan peradangan selaput yang menutupi jantung (pericardium), kadang-kadang dapat berkembang pada orang dengan myocarditis. Nyeri yang tajam di atas bagian tengah dada dapat disebabkan oleh pericarditis. Myocarditis mungkin tidak memiliki gejala nyata dalam kasus-kasus ringan.

Tanda dan gejala yang mungkin dialami oleh anak-anak ketika mengarah pada perkembangan myocarditis, antara lain:

1. Perubahan warna kulit menjadi kebiruan atau keabu-abuan
2. Kesulitan bernapas
3. Kehilangan nafsu makan
4. Demam

Pengobatan

Pengobatan myocarditis berfokus pada penyebabnya, seperti infeksi tertentu yang mungkin telah menyebabkan peradangan jantung. Coxsackievirus B merupakan tipe virus paling umum yang menyebabkan myocarditis. Telah ada pengobatan myocarditis khusus selain menghilangkan rasa sakit dan gejala lainnya.

1. Pengobatan pada kasus ringan

Untuk membantu tubuh melawan infeksi yang menyebabkan myocarditis, dokter mungkin meresepkan obat dan memberitahu pasien untuk beristirahat hingga jantungnya sudah mulai pulih. Jika penyebab infeksi adalah bakteri, maka akan diresepkan antibiotik.

Untuk menekan sistem kekebalan tubuh, virus jenis langka tertentu, seperti giant cell and eosinophilic myocarditis, merespon pemberian kortikosteroid atau obat lain.

Perawatan akan diarahkan pada penyakit yang mendasari jika kondisi ini disebabkan oleh penyakit kronis seperti lupus. Menghindari rokok, alkohol dan olahraga berat dan membatasi jumlah garam dalam diet dapat juga disarankan pada pasien.

2. Pengobatan untuk membantu kerja jantung

Seseorang mungkin dirawat di rumah sakit jika mengalami gejala, seperti detak jantung terlalu cepat atau tidak teratur. Untuk mengatur detak jantung, pasien dapat mengonsumsi obat tertentu.

Obat yang diresepkan untuk mengurangi beban kerja jantung, dapat membantu pasien menghilangkan kelebihan cairan, atau memperkuat kemampuan memompa jantung dalam kasus denyut jantung lemah.

Obat-obatan tersebut, antara lain:

a. Digoxin, yang dapat meningkatkan kekuatan kontraksi otot jantung dan cenderung untuk memperlambat detak jantung
b. Beta blockers, seperti carvedilol dan metoprolol, yang bekerja dalam berbagai cara untuk mengobati gagal jantung dan membantu mengendalikan irama jantung yang tidak teratur atau cepat

Beberapa orang membutuhkan obat-obatan hanya beberapa bulan dan kemudian sembuh sepenuhnya. Struktur Otak Pengaruhi Jumlah Teman di Facebook

London, Jumlah teman di jejaring sosial seperti Facebook tidak hanya ditentukan oleh popularitas seseorang. Struktur otak juga berpengaruh, sebab orang-orang yang punya banyak teman memiliki ukuran otak yang lebih besar di bagian tertentu.

Menurut penelitian terbaru di University College London, orang-orang yang punya banyak teman di jejaring sosial cenderung memiliki grey matter atau area di otak besar yang lebih besar. Bagian otak ini berhubungan dengan kemampuan berinteraksi dengan lingkungan sosial.

Dilihat dari ukuran atau volume grey matter, maka seharusnya perbedaan jumlah teman akan teramati baik di internet maupun di dunia nyata. Asumsi ini juga terbukti benar dalam penelitian itu, berdasarkan hasil pengamatan terhadap sedikitnya 125 mahasiswa.

Namun ada juga perbedaan struktur yang khas yang membuat beberapa orang suskes berteman di internet, tetapi tetap sulit bergaul di kehuidupan nyata. Secara kasat mata, orang-orang ini tampak memiliki ribuan orang di Facebook tetapi sering kesepian dalam kesehariannya.

Penelitian yang dilakukan lewat pemindaian otak ini menunjukkan, sedikitnya ada 3 area di otak yang tampak lebih besar pada orang-orang dengan jumlah teman lebih banyak di Facebook. Ketiganya adalah right superior temporal sulcus, left middle temporal gyrus dan right entorhinal cortex.

Right superior temporal sulcus dalah bagian otak untuk mengenali obyek bergerak, sedangkan left middle temporal gyrus menentukan kemampuan navigasi. Satu-satunya yang berhubungan langsung dengan pergaulan adalah right entorhinal cortex, yang fungsinya untuk mengenali simbol-simbol sosial.

"Tidak jelas apakah struktur otak yang seperti ini terbentuk karena banyak teman. Bisa jadi seseorang punya banyak teman karena struktur otaknya memang demikian," ungkap Dr Geraint Rees yang memimpin penelitian ini seperti dikutip dari Healthday, Rabu (19/10/2011).
